  3. A copy of identification card
  4. A copy of transcript and official Degree Certificate
  5. Two letters of recommendation (Form App02)
  6. Statement of purpose and research proposal
  7. English Proficiency Score: Applicants must submit valid English proficiency test scores from one of the following: TOEFL, IELTS, CU-TEP, MU GRAD TEST, TU-GET, or KU-EPT.
  8. A copy of certificate of Change of Name/Surname
  9. A medical certificate
 

Remark: All copies of original documents must be “certified true copy” by applicant’s signature.
